Community Health Care - Canal Fulton, OH
Community Health Care of Canal Fulton is the oldest and one of the largest of our eleven office
sites. The office is located on the northwest side of Canal Fulton
on Route 93. The office manager is Mary Ann Snyder, RN. We are welcoming new patients,
and participate in most major insurance plans. The office draws patients from Canal Fulton,
Massillon, Jackson Township, Clinton, Doylestown, Manchester, Akron, Portage Lakes, North Canton,
Canton, Green, Brewster, Beach City, North Lawrence, Marshallville, and other outlying areas.
This office admits patients to Barberton Citizens Hospital
and Massillon Community Hospital.

Providers
Rodney K. Ison, M.D.
He is a graduate of the University of Kentucky College of Medicine. He completed his family practice residency
at Barberton Citizens Hospital. He is board certified in family practice and additionally board certified in
geriatrics with the American Board of Family Practice. He is a member of the American Academy of Family
Practice. As well as taking care of patients, he also serves as President of Community Health Care, Inc.
Pamela Rucki, M.D.
She is a graduate of the Medical College of Toledo. She also completed her family practice residency at Barberton
Citizens Hospital. She is board certified by the American Board of Family Practice and is a member of the
American Academy of Family Practice.
Ajay Nair, M.D.
He is a native of the Canal Fulton area, and graduated locally from Northwest High School. He completed his
medical studies at the Northeastern Ohio Universities College of Medicine, and did his family practice
residency at Barberton Citizens Hospital. He is board-certified in family practice, is a member of the
American Academy of Family Practice, and has a special interest in Sports Medicine.
Tammy Lincoln, MSN
She is a graduate of the University of Akron's Adult Nurse Practitioner Program and Washington University's
Family Nurse Practitioner program, and is certified in Ohio as a family nurse practitioner. Tammy is
also a Certified Diabetes Educator. She is able to provide care to patients of all ages. She also
coordinates clinical studies at our facilities.
