Hartville Family Physicians is the fifth office of Community Health Care. The practice joined Community
Health Care in April, 1996. The practice has since that time moved to new, larger quarters located
at 855 W. Maple St. in Hartville, and the phone number is (330) 877-3616. The office manager
is Jane Mahaffey.
Hartville is located in Stark County, in Northeastern Ohio, not too far from Akron and Canton. The
practice draws patients from Stark, Portage, and Summit counties, from the areas of Hartville,
Lake Township, Uniontown, Green, Springfield Township, Green, North Canton, Canton, Plain Township,
Mogodore, Suffield, Brimfield, Kent, Alliance, and other surrounding communities. This office
admits patients to Aultman Hospital and
Mercy Medical Center in Canton.
Hartville Family Physicians has been a pioneer in our geographic area in the use of electronic
medical records via eClinicalWorks software.
Appointment hours are: 8am-8pm Monday through Thursday, 8am-5pm Friday, and Saturday 9am-Noon.
We are welcoming patients of all ages, and we participate in most major insurance plans...call our office for details.

Providers
Martha Klipec, M.D.
She is a graduate of The Northeastern Ohio Universities College of Medicine and completed her Family Practice Residency at Akron General Medical
Center in Akron, Ohio. She is board certified in Family Practice by the American Board of Family Medicine, and is a member of the American
Academy of Family Practice.
Paul Lenz, M.D.
He is a graduate of the University of Cincinnati College of Medicine and his Family Practice Residency at Barberton Citizens Hospital. He
is board certified in Family Practice by the American Board of Family Medicine, and is a member of the American Academy of Family Practice.
He also attends extended care patients at Altercare of Hartville and Altercare of Louisville. He is an avid runner and is training for
the Triathalon.
Jon Seager, M.D.
He began his full-time practice with us in August, 2002. Dr. Seager is a graduate of The Ohio State University College of Medicine, and
received his family practice residency education at Barberton Citizens Hospital. He is board certified by the American Board of Family Medicine.
Louise Kolarik, M.D.
Dr. Kolarik went to medical school, in England,
at Leicester-Warwick Medical School and did her family practice residency at Aultman Hospital in Canton.
She is board certified in Family Practice.
Angie Gager, FNP
She joined Community Health Care in August, 2000, and has been with Hartville Family Physicians since 2007. She is able to provider
physical exams, illness and wellness care, health education, chronic disease management, and much
more.
