Rodney K. Ison, MD

Rodney K. Ison

Dr. Ison is the founder of Community Health Care, Inc. He is Board Certified in Family Medicine. He graduated from the University of Kentucky and completed his family practice residency at Barberton Citizens Hospital (Summa Barberton Hospital). In 1986, he opened the first Community Health Care, Inc. office in Canal Fulton, Ohio. He is a member of the Ohio Academy of Family Practice, the American Academy of Family Physicians, and the Summit County Medical Society. In 2000, he received the Outstanding Faculty Award from Northeastern Ohio Medical University.

Pamela Rucki, MD

Pamela Rucki

Dr. Rucki is board certified in Family Medicine. She graduated from the Medical College of Toledo and completed her family practice residency at Barberton Citizens Hospital (Summa Barberton Hospital). She joined Community Health Care, Inc. in 1988.

Ajay Nair, MD

Ajay Nair

Dr. Nair is board certified in Family Medicine. He graduated from Northeastern Ohio Medical University and completed his family practice residency at Barberton Citizens Hospital (Summa Barberton Hospital). He graduated in 1990 from Northwest High School in Canal Fulton, Ohio. He enjoys providing care for sports related injuries and is very involved in the local high school’s sports and academic programs. He joined Community Health Care, Inc. in 1999.

Dr. Nicole Wade, DO,

Dr. Nicole Wade, DO

Nicole Wade, DO grew up in Stark County, Ohio and attended Ohio University College of Osteopathic Medicine in Athens, Ohio. She is Board Certified in Family Medicine and has gained her post graduate education in both Kansas and Wisconsin. After training she combined her passions of travel with medicine, working in various regions of the United States adding Arizona, New Mexico, Hawaii, Alaska, and Washington to her experience. She has worked in the Urgent Care aspect of medicine for many years and comes to Community Health Care to provide support to the Primary Care Providers. She is available for acute care needs, time sensitive concerns, and same day appointments. She joined Community Health Care, Inc. in 2025.

Tammy Lincoln is a Board Certified Primary Care Nurse Practitioner. Tammy received a Master of Science in Nursing from the University of Akron. She has clinical expertise in diagnosing and treating health conditions with an added emphasis on disease prevention and chronic care management. Tammy joined Community Health Care Inc. in 1999.

She has clinical expertise in diagnosing and treating health conditions with an added emphasis on disease prevention and chronic care management. Jenna joined Community Health Care, Inc. in 2017.
